Indicate the situation in which you find yourself:
- Download the document TA. 6 registration in the Social security system.
- Indicates ‘ 151 ‘ in the field ‘ number ‘ and ‘ Asepeyo in ‘
denomination ‘ of section 7.1 entity of occupational accidents and
diseases.
- If you want, also, that from Asepeyo we cover the benefits of
common contingencies, mark with an X section 7.2 mutual.
To expedite this process, we offer you the form With these two fields filled in.
- Please give this form, along with the rest of the necessary documentation, to the General treasury of the Social security.
- You must apply for the download to your current mutual, with at least one month in advance of the expiration date of your association document. Use our Low Application Model.
- The current mutual must issue the certificate of cessation and get you there.
- If the company has a company committee or staff delegates, they must give their opinion by completing the Mutual Change Report.
- If the company does not have a company committee or personnel delegates, it must complete the Mutual change report without trade union representation.
- It delivers this documentation in the General treasury of the Social security, with a minimum advance of 10 days to the expiration date, next to the certificate of Baja. Once this procedure has been made, the change will be effective from the day following the date of the drop that appears on the certificate.
